Carpal Tunnel Syndrome
What is Carpal Tunnel Syndrome?
Carpal Tunnel Syndrome (CTS) is a common condition caused by compression of the median nerve as it passes through the narrow carpal tunnel in the wrist. This compression can lead to numbness, tingling, pain, and sometimes weakness in the thumb, index, middle, and half of the ring finger .
Anatomy & Causes
The carpal tunnel is a rigid passage formed by wrist bones at the back and the transverse carpal ligament at the front, which holds the median nerve and flexor tendons in place . When the tunnel becomes narrowed or tissues swell, the median nerve is squeezed, leading to symptoms .
Common Symptoms
- Numbness, tingling, or “pins and needles” in the thumb, index, middle, and radial half of the ring finger
- Aching or discomfort in the wrist or forearm, often worse at night or when gripping objects
- Difficulty with fine motor functions, like buttoning clothes or holding small items
How Is It Diagnosed?
Diagnosis is usually clinical—based on your symptoms and examination. Tests such as Phalen’s or Tinel’s sign may be performed. Occasionally, nerve conduction studies or ultrasound are used to confirm the diagnosis or evaluate severity .
Treatment Options
Non-Surgical Management
- Wrist Splints, especially at night, to keep the wrist neutral and reduce pressure on the nerve
- NSAIDs for symptom relief
- Corticosteroid Injection into the carpal tunnel, which often provides significant short-term improvement
Surgical Treatment
- Carpal Tunnel Release: A small incision is made in the wrist to cut the transverse carpal ligament and relieve pressure on the median nerve. It is typically performed as a day-only procedure. Most patients achieve excellent symptom relief with minimal complications .
Post-Surgery Care
- Encourage gentle finger movement soon after surgery
- Dressing and stitches typically removed after 10–14 days
- Occupational therapy may help improve strength, flexibility, and function
- Most patients return to normal activities within a few weeks, with durable symptom relief
Risks & Outcomes
Carpal tunnel release has a very high success rate. Rare complications may include:
- Persistent numbness or discomfort
- Scar sensitivity or infection
- Incomplete symptom resolution if nerve damage is advanced
